BE PROPERLY PREPARED FOR YOUR LIVE SCAN or FINGERPRINTING APPOINTMENT:
You will need to provide the following three items:
1. BCII 8016 “Request for Live Scan” Service Form – Prior to your appointment, please fill out the BCII 8016 form completely.
2. Social Security Number – You must list your Social Security Number on your BCII 8016 Request for Live Scan Service form. This is a DOJ and FBI requirement. If you do not provide your Social Security Number, you will not receive service.
3. Acceptable Valid Photo ID – In order to receive Live Scan services you must present a valid photo ID to the technician. Expired identification cards will not be accepted. Following are examples of acceptable forms of photo identification:
